NURSING SERVICES

An essential part of our team is the rehabilitation therapists.  Our new state of the art rehabilitation unit allows our patients to receive therapy in comfortable surroundings. The in house therapy department works as part of the team to insure each patient reaches their potential and recover as quickly as possible for a speedy return home.

 

Our dedicated team of nursing staff comprised of RN's,LPN's,CMT's and CNA's use a holistic approach with each resident. We evaluate and assess each individuals needs and provide the care that is necessary to promote healing and recovery. Our nursing team offers a wide variety of specialized services such as Infusion therapy, Wound care, Pain management, skin assessment and Cardiac rehab to name a few. Our Professional staff works together to create a care plan designed for each resident to reach his or her maximum potential in as little time as possible.

Our Social Service Department works with both the resident and family to promote the comprehensive multi-disciplinary approach to address all aspects of the resident's needs.

Whether a resident requires long-term, short-term placement or respite services, our social workers begin individually assessing a patients social needs such as adjustment to their new environment, preferences in activities and involving the family in the routine of the facility. For resident's who require short-term placement we offer a team approach in regard to discharge planning.

We offer referral services to community resources, such as VNA services, Meals on Wheels, Life Line, Elderly Day Care Services and DME companies.  Should a resident require Long-term placement we provide comfort and support to the resident and activities that foster new relationships and friendships.
